Application Form of account holder for account opening signed by the person authorized to open accounts
Notarially certified copy of the Extract from the Trade Register of the host country of the corporate non-resident or other equal evidence of legal status in accordance with the legislation of its host country. The extract should be issued within the period not exceeding a year before application form concerning the opening of account is submitted
Notarially certified copy of the Articles of Association and (or) Articles of Incorporation (other equal evidence of legal status in accordance with the legislation of its host country) pursuant to which it acts
Copy of the permission of the Ministry of Foreign Affairs of the Republic of Belarus (Regional Executive Committee (Minsk City Executive Committee) for the opening of representative office (branch) (in case account is opened to separate business unit) which is certified notarially or by the account holder
Copy of the Regulations on separate business unit or other equivalent document which is certified notarially or by the Bank’s authorized officer
Specimen Signatures Form. The Bank’s officer certifies signatures within the form to be original to signatures specimen

It is required to additionally submit documents evidencing the powers of persons whose signatures are certified within the Specimen Signatures Form (contracts, orders, etc.)
